What do you need to bring to a reservation?

I, and my family of 3, have a reservation at 730pm at the Biergarten Restaurant in Epcot. I made the reservation several months ago online, and as a result can't remember anything about it!
Do I need a confirmation piece of paper, or can I just show up, say my name, and get the table?

You don't need anything. Just show up and they should have your name on their list. Very occasionally they can't find your reservation so if you have your confirmation number it will help if there is a problem. But that is very unlikely to happen.

It's always good to have your confirmation number handy. There have been reports where they couldn't find it by name and this helped immensely. If you don't have your confirmation numbers anymore, you can call Disney Dining and have them look them up for you no problem .
