Can you upgrade two park hopper passes each with 2 days left to 1 single annual pass?

No you can't. Only one old ticket can be applied to each new ticket.

Correct - you don't really "upgrade" a ticket to an AP, you trade it in for monetary credit towards the purchase of an AP. And Disney only allows you to apply credit from one ticket toward each AP purchase.
