My friend, who has a son with ADHD, was told on another message board that he would qualify for unlimited fastpasses along with five guests. I had never heard of this before. My mother has MS and has difficulty standing up for long periods, so this would be great for her. I was wonder what conditions qualify for this accommodation and what the procedure is. Has anyone had experience with this?

This is a common misconception about how disabilities are handled at WDW. GACs are not given for a diagnosis, they are given based on accomodating difficulties that would make the guest otherwise unable to participate in the attraction. What accomodation is given, if any, is very much dependent on the specific difficulty that the person would experience and the way the GAC is handled can vary by attraction and time of day and some attractions have no accomodations. GACs are not intended to shorten wait times and in fact will sometimes make wait times longer. There's more information about GACs in the stickies at the top of this forum.

For your mother, the standard response for mobility issues is to rent a mobility device such as an ECV. Disney has no ability to shorten distances. An ECV would help her immensely though. Think of it this way; even if you didn't have to wait long in lines (which there isn't a GAC for), she would still have to walk anywhere from 6 to 10 miles per day between attractions and through queues. You REALLY want to consider an ECV. I would recommend renting from an offsite company so that she'll have it for outside the parks as well, plus it's significantly cheaper renting from an offsite company. There's information about renting in the stickies at the top of this forum.

Agreeing with the previous poster.
For more information about Guest Assistance Cards, see post 6 of the disABILITIES FAQs thread. That thread is near the top of this board or you can follow the link in my signature to get there.
