If you apply for a position, but are offered a different position with, potentially, physical requirements you're unable to perform, can it affect your unemployment?

You mean unemployment benefits? No it shouldn't. Just answer the claim questions honestly. The worst that will probably happen is someone from the Department of Labor will call to understand the details. Happened to me not terribly long ago.

I would let the employer who offered you the job know that you might not be able to preform the tasks of the job and see if their able to accommodate. Now if you don't want the job for whatever reason that's a different story but if you do I would disclose your concern and see if they can accommodate.
