I have a store bought one and the fields that you already don't have listed are;
Where we stayed
Special purchases
Places we visited

I thought about making my own. But I just bought a pre made one . I believe the intent is to use one per trip. But I will plan on using one per day. Mine has the front page the questions, then the back page is blank so that is where I will write about the entire day
