Travel docs for DNephew

DH and I are bringing DNephew(5) on a trip to WDW in February. I was thinking of getting a letter from his mother notarized saying that we can bring him to the hospital, etc. in case of emergency. Is there anything else I need? A birth certificate to get him through security and onto the plane? Thanks for any help. It's our first time traveling with a child. I don't know who's more excited, him or us!:banana:
 
We have taken 3 nieces and 1 nephew to WDW. Even though they have different last names we were not questioned at the airport about it or asked to see any form of ID. All anyone at the airport was concerned about was that they were ticketed passengers (all over the age of two of course so they had to be). Of course this was domestic travel w/in the U.S. - I'm sure it would be different if we were taking them out of the country.

My suggestion is to use a limited Medical Power of Attorney form like this one I found online http://www.doyourownwill.com/poas/cpoa.doc, and then have it notorized.

That should have you covered in case of (God forbid) a medical emergency should arise. We, thank goodness, never had any problems. All the kids enjoyed their trips to WDW. I hope you and your nephew have a great time!
 
I don't have any advice on what you would need but I wanted to say how sweet y'all are for taking your nephew with you!!!
 
Thanks for the replies. That power of attorney was just what I was looking for.
 

We sent our DS9 to Costa Rica with his Grandparents to visit family last spring. We sent him with an letter authorizing them to take him out of the country and a temporary power of attorney. Since he was leaving the country the requirements were a bit more stringent. But you'll definitely, definitely want a power of attny. Depending on how cautious you need to be - permission to take him over state lines. (That one sort of depends on the family situation - you got anyone who might call him kidnapped? - bad divorce, etc.)

let me see if I can find that power of attny... found it. It's specific to the State of California. Your state may be different. Looks like my husband purchased it for about $8 online somewhere. I also have the Affidavit of Parental Concent to travel outside the US. Both are notarized documents. Send me a PM if you want them.

Don't forget to get his medical insurance card. Hospitals are very strict about that. My brother just requested an extra and I keep it in my wallet with a letter for any emergencies with my nephews.
 
Thank you all for your help. I really appreciate it. I will be using the POA form that all4fun posted, and will definitely get a copy of his insurance card just in case. I knew all you Disers would have the answer.
