My family and I are planning on staying at the Swan in December. However, we are also booking a Disney resort package for that time in hopes that free dining will be offered and then we would cancel our Swan hotel. If we have already booked our ADRs using our Disney resort reservation and then cancel that reservation to stay at the Swan, would we be able to transfer our ADRs to our new tickets?

I don't believe your ADR's and room booking are linked in any way. You can make the same dining reservations without a room, right?

I think your ADRs are booked to your park tickets, right? So if I cancel my Disney resort package, I also have to cancel the tickets associated with it and buy new tickets separately while staying at the swan. I should mention I'm not booking the swan through Disney, I would be staying on credit card points.

ADRs are not linked to anything. You can make them without tickets or ressi. Last time we went I made ADRs before I even made our package ressi.

The only link a resort reservation has to ADRs is the ability to make all of your ADRs (up to 10 days) at the 180 day mark. You can make ADRs without a resort reservation, but you would have to make them every day, rather than all of them at once. With that said, if you book a package now and cancel it later, your ADRs will not change; however, the tickets would be cancelled along with the resort and would just buy your tickets separate.
