Yes. A cruise booked with a placeholder discount can be transferred and maintain the discount as long as the new cruise departs within 2 years of the placeholder’s original purchase. If you cancel a cruise, though, you will lose the placeholder discount.
Also, if the cruise has been transferred to a travel agency, that agency must make the transfer.

Technically no, the placeholder can't be transferred. Once the placeholder is 'used' to book a specific sailing, the placeholder goes away. What you can do is move to a different sailing, and the benefits of the placeholder will move with the sailing (as long as it's not a placeholder blackout date). But be sure you are moving to a different sailing, not cancelling and rebooking (or you then will lose the placeholder benefits and your $250 or $500 deposit will be refunded).
Yes, you can change dates, ships and itinerary. The key word is "change" -- don't cancel the original placeholder reservation, you change it (to new dates, different ship/itinerary, etc.).
As a PP mentioned, a "placeholder" cannot actually be transferred. A placeholder is in itself a reservation, whether attached to a specific cruise or still open. That reservation number holds the placeholder benefits, which cannot be "transferred" to any other reservation number.
