I don't know if it would work but one of the cutest costumes I ever saw on a little one was "a bunch of grapes"... they put a green stocking cap on for the stem and attached lots of purple balloons to an old t-shirt... can't remember exactly how they attached them but it was adorable...
I'd think you could do a pumpkin in a similar way... if you didn't want to use actual balloons you could make little orange balls from cloth and batting and sew them on to an orange t-shirt... then add a few black ones to make the jack-o-lantern face.
