Wait until you arrive in WDW and go to any Guest Services at the parks or DTD and get your card. Since it expires 1 year from the date it's issued, you don't want to order it by mail and have it start losing time before you get to use it.

I got that list from the Florida Resident's section of the disneyworld.com website. The link is too long to post, plus you have to enter your zip code to get to that section, so just go to
http://www.disneyworld.com, click on Florida Residents (it's in tiny letters at the very top of the page on the right, directly above the search box). Enter your zip code (I used MILs) A page will open that says "Welcome Florida Resident". Below that choose "Food and Beverage Discounts"....then on the next page it will begin to describe Disney Dining Experience (the new name is Tables in Wonderland, just changed this week, but not all the website pages are showing that). There is a "show more" tab and that's where I found the list I posted above.
