Standard form for travel with children for authorization for customs?

Canadian government website has standard form for travel with kids without parents (authorization form).

I found this form on the boards and kept the link, but then tried the link few days later and no longer worked.

Does anyone have that standard form or know where to get it from?

We're traveling October 4. Thanks for your help
 
I am meeting up with dh in Anaheim in November and will need this. I came out of my cave and realized just recently that we had to have this notarized (i had no idea lol)
 

I am traveling with my children next Saturday but my husband is not going .

I need this form or is it just a back up just in case they ask?

My husband has crossed the border with my son for hockey games before without it. And didn't get asked.

If you are driving I am unsure. If you are flying absolutely. It really depends on where you are flying from. You cannot fly out of Calgary without it but I have read some people fly out of Ontario and never get asked.
 
I am from the Toronto area but am flying out of Buffalo.....I have already crossed the border...:lmao:

I forgot that won't help because hubby is the one dropping us off at the airport.....so won't get asked at the border either.

Well they ask at the airport I wonder. We are flying Airtran. That is what Disney put us on.
 
You may get away without having the proper authorization form -- many people cross the border back and forth without it with no hassle -- but you just need to be aware that it can be asked for when entering the states as well as entering back into Canada, and you can be delayed or even refused entry if you don't have it.

Think of it this way -- day to day, you could always leave the house without your drivers license, right? I mean, how many times do you ever have to show it? But, if you don't have it on you and it is asked for.....................

This is an issue that I feel very strongly about -- it makes me angry that border guards don't ask for it each and every time. My husband is a police sgt here in Toronto and has dealt with cases involving parental kidnapping, where the child was taken across the border with no questions asked. It's a horrible thing to happen to a family and it could so easily be prevented if the customs guards would be more vigilant!

But if hubby is driving you across the border and then picking you up in Buffalo and bringing you all back, you have no worries as you're crossing the border as a family :) I doubt you would be asked at the airport, as you will only be on a domestic flight.

Are you taking a Disney cruise or just flying down to the parks? On the cruise, you may be asked for it as you are travelling internationally.
 
Just going to the parks this time. Having a Dis meet there with some friends from the community board. They picked a time hubby couldn't get away from work.

I am very nervous about traveling with out hubby. We have done it many times but it still is scary. Since it is a domestic flight there isn't a custom form either to fill out on the plane right. That was hubby's job.

We have done the cruise before. Don't think hubby would miss one of those...:lmao:

I can understand asking about kids crossing the border and who they belong too.

That's I am sure I will be fine once I am on that plane on next Saturday with the kids.

Who wouldn't be excited to be going to Disney right...:rotfl:

We have flown separately on our last 2 vacations. Both from Winnipeg. I was asked for the form in the morning (flying with DD4) and DH was not asked for it 6 hours later (flying with DD11). You might as well have the form...
