SR - How to cash out?

I've finally earned enough credits to get a gift card but I get this screen that says

So I left it all blank and it said I had to fill it out. What do I write? I also am not happy with SR lately.

It's getting really hard to get credit on there lately. I've done numerous "free stuff" and have cleared cookies and gone through a different browser but no luck. I even have confirmation numbers and passwords to their site but I guess it doesn't matter.

I was doing really good for awhile and now nothing!!

From the redemption page......If you are redeeming for gift certificates, please insert "n/a" in each blank below and the gift certificates will be sent to the address that we have on file for you.

So just place N/A in all the boxes and hit place order.

what do we do if we are getting disney dollars? the same thing?

Sometimes even though you have cleared your cookies, you may have signed up for an offer under a different name on a different reward site and its held in the advertisers system and they know and will not credit you for it. There is nothing SR can do about this, or any other reward site for that matter. When you do the free offers, you run a much greater risk of not getting credits. They are often masked in different names. If you go to a lot of different sites and do this, be prepared. Also, some browsers do not allow for crediting, so sometimes that is a problem. Have you taken any surveys or done any shopping and not got credit for those at SR? To cash out, just go to the redemption page and click on the redemption and fill out the form the best that you can for what you are cashing out for. If you are undecided, you can always write in the comments and we will see that and if we have any questions, we will contact you.

They only shopping I've done so far is through e-bay and I haven't got credit for that and I bought it 4/21.
