I generally make manual charts and play around with a number of different combinations of what parks at what times, trying to be the most efficient and see the most special events as possible (parades, fireworks, etc.).
We aren't big foodies and generally feel that TS meals take a lot of time out of touring (which is already limited by nap break with little kids). Last trip we did 5 TS meals and it was too many for us. This trip I only have 1 planned so far, but I'm considering adding another one. (We are staying at a condo, so we can have decent food for breakfast there, etc.).
We always get hoppers, for the flexibility.
With respect to what to do in the parks each day, I have a general idea of what I want to accomplish in the first 2 hours each morning. So, I'm an am commando. After that, I still like to move from attraction to attraction, but I'm more flexible (other than at DHS and AK when the show schedules are more limiting).
I also make sure that each person in the party gets to do something each day that is high on their priorities. For example, for our DHS day I believe that the priorities are as follows:
DH: Toy Story and ToT
DD7: RRC
DD4: Phineus and Ferb M&G, Little Mermain show
ME: New Star Tours, characterpalooza, Osbourne Lights
Similarly, when we went to DL last summer with a party of ten, I planned so that each person would have 1 or 2 special things geared towards them during the 4 day trip. Here is a sampling:
DMom (my mom) - the Aladdin theater show
DNiece (9) - ToT, California Screaming
DNephew (3) - all things Toy Story (Buzz, Toy Story Mania, etc.)
DSister (my sister) - fun shopping (but she had to wait until we'd done our morning routine)
DD (who was 6 at the time) - meeting new characters
DD (who was 3 at the time) - meeting Ariel
Me - riding Dumbo, experiencing attractions unique to DL
DDad (my dad) - the shooting gallery
DH - Space Mountain
DNiece (6) - getting ears

This way if we get separated or one of us has to leave with sick child he knows where to go and how to get there.
I bet that confused everyone like it confuses my husband 
) we try one time to go by eact schedule,but most of the time where just going by that schedule and it did not make it very fun. We ended up throwing it out. Now we just use it as a guide and if we stray from it no big deal!!.
