Shares - Bought with Selftrade now cannot find shareholder form etc

Hello, yes it is me again!

Well I have bought the shares - halfway there!!!

I have printed off the contract note.

Now I cannot find the shareholder form on the disneyland paris website to complete and send off - Can anyone help PLEASE

There is always one that causes trouble and I think it might just be ME!!

I have completed the form and now have to send off a signed copy and my contract note.

The fax no is showing as +33 (0)1 64 74 56 36

Can you please advise do I put in the above number (starting 33) or do I just put in 0164745636??
