I can't get it from either iPhone/iPads or my laptop win10 either.

It only worked on one netbook for me, I managed to email the form and proof of shares at the beginning of the month and received my card yesterday. I had emailed them to send me a form to sign but they did not reply to that, they only replied when I sent all the stuff in.
