Well now, from what I remember when I used to work behind a bar many moons ago, when somebody asked me for a Pint of Shandy it was always half beer or lager and half lemonade, if they wanted a lemon top or beer/lager with a dash it was 90% beer/larger and 10% lemonade.
Although some people did occasionally ask for a weak shandy, hence less beer/larger.
Im sure it's still the same, and no there were no other ingredients that we used.

Actually a proper shandy is half of bitter and half of ginger beer. It was only in the 1970's that lemonade began to replace ginger beer. For anyone not having tried a shandy with ginger beer instead of lemonade it really is a far superior taste. Bitter/Lager tops should only have a splash of lemonade not 10% that would dilute it too much.
I like shandy made with ginger beer instead of lemonade - but last time I asked for one the bar man made it with ginger beer in place of the beer!!
Ginger beer is nothing like root beer, it's a bit like ginger ale but hotter(spicier). We used to make our own when I was younger and kept a ginger beer "plant" in a sweetie jar in the cellar - the ginger beer we made from that was quite firey.
