In almost all cases, you'll need to send SAT/ACT scores and your high school transcripts if you haven't already attended an institution for a higher of learning for a period of time (as determined by the institution you'd be interested in applying to). This applies regardless of your age. There are probably some exceptions, but there is no universal rule.
The exception here is community colleges, which often just require evidence of your high school graduation or attaining your GED. You may however be subject to a placement exam at the college itself, so that you can be properly placed into math/english sections. Also note that some programs do require high school transcripts.
It depends on the school. Generally speaking, though, you would need to be over the age of 23 before it is even considered. You WILL be required to take some type of placement exam prior to registrations, however, even at Community College level.
