What should I do with these? Do I need to bring a cooler and pack them up? I vaguely remember someone mentioning to tell bell services if anything needed to be refrigerated, but I don't know what to do with it other than that.
Just bring one of those soft sided sturdy cooler bags. You can pack it in your luggage pretty easily. Bring some ziplocs you can fill with ice to keep everything cold. Bell services will move anything you want (for the most part) but don't have the ability to keep things cold. When they move your luggage they load it up on a truck/trailer and make stops at several resorts.

Unfortunately, in my past experience, If you are staying or storing anything at Port Orleans Riverside you may find that you are told that they don't have room to refrigerate your beverages and you may find that they have crammed so much stuff in the refrigerator that your yogurt packages burst and leak all over the other items in the bag from CM's SHOVING stuff in the undersized fridge.
It's a real "Lord of the Flies" situation over at POR bell services with too much stuff that needs to be stored and not enough room to store it all. Don't count on having all the stuff that you asked to be stored returned to you either. Our cases of beer and soda went missing and were only returned when we called the manager to make a big deal over it.
In essence it depends or where you coming from/going to and what the storage situation is at those sites. Good luck but don't count on anything resembling good customer service if you are using Bell Services at POR. We experienced bad bell storage service on multiple visits.
Well that's kind of disturbing. I'll actually be transferring from POR to AoA. Possibly. It's actually sounding more and more like it may be too much trouble, might end up just staying put!

As others have mentioned, I would bring a soft sided cooler and use that to keep items cool while they're transferred. If you have medications that need to be kept cool, I would ask down on the Disabilities board for suggestions. Luckily, things like drinks (soda, water) can be left out of the refrigerator and then re-cooled when you get to your new resort. Some food items might be less forgiving, so I'd pack those in the cooler with lots of ice. Just make sure the ziploc baggies are closed/sealed and do not leak. I'd check-in to the new resort and then head back to your room as soon as it's ready. Then you can move your perishable items from the cooler into the new fridge.
