first button you hit is rent movie with code then enter the code

You can also do it at checkout, there will be a button that says enter a code, every Monday a code is sent out, but it is only good for Monday until midnight

No, you don't need an account.
You just use the code at the checkout, but you still have to scan a credit/debit card in case you don't return it and you accrue fees.
