Recommendations for a good first aid kit for a survival pack?

Building a survival pack to keep in my car in an emergency, will include emergency food/water, lighting in the form of both flashlights and glowsticks, firestarting, multi-purpose tool, and of course a first aid kit.

But I'm having trouble finding a medkit that has anything beyond basic supplies. I need one that's compact and will fit in a medium sized backpack with everything else, but it also needs to have supplies to treat more serious wounds in dire circumstances, namely sutures for lacerations and splints for fractures and broken bones. Compact I can find, but nothing I've researched seems to include the sutures or any sort of surgical/trauma supplies for severe injury.
 
I would start by determining exactly when/how you think you would need to use this so you can include the appropriate items. Do you plan to use it when hiking in some remote area of the woods or during an evacuation if you live someplace where hurricanes are common? If you live out west, then planning for a major snowstorm would likely include different items vs. living in Florida where a hurricane might occur. I don't think there is a one-size-fits-all type of emergency kit.

How much emergency food/water would also seem to depend on the expected duration of whatever situation you are planning for. Unless someone in your party has medical training such as a doctor/nurse, I am not sure how you would ever make use of some of the items you mentioned even if you carried them around with you.
