Rebate on appliances ...kohls

I ordered 3 appliances for $19.99 ea on line from Kohls this am. Each appliance has a $12.00 rebate. How do I go about getting my rebate form in order to get my rebate.

Will the form come in the box with the appliance. I could not find anything on line regarding printing out a rebate form.

It will not be in the box with the item. You will need to save the packing slip and you will also need to send in the UPC code from the item's box.

You need to print it out. On the item description there is a link for a rebate form, which is good for all the eligible appliances. You will need one copy for each appliance purchased.

The rebate form says you have to have the original packing list or rebate receipt. I ordered 3 appliances online, so I'm guessing they'll ship together and I'll only get one packing list. Can I send the three rebates in together with the one packing list?
