Question for anyone who has taken "borrowed" kids on DCL

In our docs, there is a form that needs to be filled out by the parent or guardian and notarized for the kids we are taking. Is this new?
Will a letter from the parent that is notarized do the same thing, or does this "card" have to be completed?

We took a friend along for our DD last year and plan to do the same next May. Yes, you do need to fill out ALL information given to you from DCL for this person to go along. YES, you do need to have that letter from the parent(s) stating that they authorize you to take them out of the country and it DOES need to be notarized. I wouldn't skip over anything they give you to complete for this purpose, or you may have trouble when you get to the port terminal. Also, remember the passport or certified birth certificate for that child too.

I have been a great proponent of taking someone along for our DD. We had gone on the Magic on one of its inaugural sailings, and it was just DH, DD and me and it was less than happy. Last year, with her friend along, they spent more time without out us than with us. They loved Common Grounds (they're teens) and we met each evening for dinner and talked about our day. Of course, in the ports we visited, we were always together for excursions, etc.

We brought our DS17 friend with us on the Magic on 8/3. We did not have any special forms for him in our docs....he had his own "booklet" with his name on it...like any other passenger. We filled that out and brought along a notarized and signed letter by both his parents that we had permission to take him out of the country. They also included in the letter than we had the right to authorize medical care should he need it. We also carried a copy of the boys insurance card with us.

We had no problems while onboard. I did have to sign him out off the ship when him and my son (both 17) were off the ship without us.

Thanks! I thought a notarized letter would be fine, but there is actually in the docs (each of us have a card like packet) an item that says fill this out and notarize it for parental permissions. I was wondering if a letter was enough.
I guess we will do both.
