Question about US Airways CC companion passes

When using the companion passes with the US Airways cc, I think I read somewhere that the full price ticket must be at least $350. Is this true?

If so, do you have the option of buying business class to bump up the price to the $350 mark? If you do, will the companion passes also be business class or would they have to be coach?

It's $250 and is only for coach class travel. Hurry and use it by the end of the year. They are elimintaing them as well as the US Airways CC.

I haven't applied for the card yet, but the online offer still mentions the companion passes. If I apply for the card now, how long does it take till I have access to the passes? I was planning on using them in December so I'd definitely get use by the end of the year...

P.S. my main motivation for getting the cc is the companion tickets. We don't fly much so don't rack up FF miles, but I would love to have the companion ticket perk for at least one trip...
