Question about docs

Sorry if this has already been answered somewhere before! This is our first cruise and I just want to be sure I answer correctly so I don't hold up the line! We will be on the 7 day eastern.

Is the Bahama form for CC?
The city in which we boarded ship- Cape Canaveral?
Travel agency-We booked thru allseas, but not sure to put that or DCL?
Is there a form for St. Martin or St Thomas?

I put down All Seasons & no form for St. Thomas & St Marteen. You will have to go through customs at St. Thomas, but it was really painless.

Yes the form is for CC/Bahamas, you can use Cape or Port Canaveral, no forms for either Saints, just use whatever travel agency you went through. No problems.
