Purchasing a DVC members Annual Pass?

How do I purchase an Annual Pass via the Members Web Site or must I go through the AP web site? My pass has expired and I would like to find the correct order form that acknowledges me being a DVC member thus subtracts the $100.

That is the site I was looking to find. Now how do I put the correct name on the certificate, ie my son's? If I call MS would they identify the certificate correctly?
