Priority seating-Chef Mickeys

Help please
I've done this before so why can't I remember how?
I want to book Chef Mickeys for our Jan 2005 trip and can't remember the telephone number!
I also remember there being a 'priority seating' planner somewhere........
Any help would be much appreciated.

At least that's what is says on the bottom of my sheet!! It also gives you the numbers to use.

We called Disney last night and made reservations for chef Mickeys' and the Princess breakfast at Epcot.
