Do any other resturants offer a personalized menu or just Coral Reef?

1) Any sit-down restaurant with paper menus can offer personalization.
2) Usually, laminated menu places are not able to be customized.
3) The staff just needs to be willing at that time.

So has anyone else gotten personalized menus anywhere else? I'm not even sure who has paper menus.....but does anyone know!

I did NOT request one, but received one at Narcoosee's

My birthday was noted on my ADR and they did one for everyone in our group. It was a fantastic surprise.
