Two quick questions about personalized menus

1. How far in advance should I call to ask about having one?

2. Do all restaurants offer this service or just a select few?

1) All table service eateries can do an individualized menu. *
2) It just depends upon if they want to.
3) If you get a CM who doesn't want to or is too busy, then no.

* Usually, eateries with laminated menus will be resistant to requests.
