Thanks for your input, everyone! 
The dr was concerned about DS's weight when he was a baby, but over time DS did start to gain weight at his own pace and we went from frequent weight checks to just regularly scheduled check-ups.
DS had his tonsils out recently, and I had heard from many parents that their kids' appetites increased a lot after their tonsillectomies. I had hoped that the same would be true for DS, but it was not. I mentioned this to my pediatrician and this is when he mentioned Periactin. He said it was completely up to me, but it might be worth a try to see if it helped get his appetite going.
I told him that I wasn't going to do it right now because I couldn't see giving him medicine that he didn't need. But then I start to worry about him being underweight. As a baby, he had some gross motor delays which they attributed to him not getting enough calories to have the energy to pull up, sit, crawl, etc. (we had to make his formula with an extra scoop of powder per bottle so that he would have more calories per ounce).
As for his motor skills now, he is still a little bit behind his peers, so I worry about not doing everything I can to make sure he can catch up.
I think I would be less concerned about his weight if he ate well (my other DS eats pretty well and is super skinny, but I know he's eating). Food doesn't have much appeal to DS4, I mean, the kid doesn't even like ice cream, chocolate, mac & cheese, or most other kid favorites (I don't know where I got him - I consider chocolate a necessity
).
So, for now, I think we'll wait and see. He's growing fine (54th percentile for height), but the disparity between his height percentile and weight percentile concerns me.
