Parental Consent form

After posting a few days ago http://www.disboards.com/showthread.php?t=1486775 I decided to try find a consent form suitable for taking someone elses child to Florida. Searched for hours could not find anything. I expected a .gov site to have the appropriate form but I could not find it.
I decided to create one, started with one I found then adapted it with bits from other sites.
I have put a copy on my website if anyone wants to use it, just copy and paste into word and print off. Can easily be changed if wording is not suitable for your needs.

Thanks very much for this Wayne, really helpful.

I'll be taking my half brother in August (his first trip) and we may need something like this. As he is a minor and I am now married so different surnames, you can never be too prepared with US immigration.
