My DD (13) and I are cruising without the rest of our family. I remember a post about a form that has to be filled out before the cruise with the signature of the spouse that is not cruising. This form gives permission for the cruising parent to take the child out of country. Where would I locate this form and when is this form presented?

There is no official form. US Customs and Border Protection recommends, but does not require, that you have a notarized letter signed by the absent parent stating that they grant permission for you to travel with the child outside of the country. Here is a link to the info:

The Minor Authorization Form is for a child traveling without either parent/guardian -- such as bringing a friend, child traveling with grandparents, etc. As mentioned above, there is no form required but a notarized letter is recommended.
