in 2002. It was around $220 for the 4 of us. This was based on the cost of the trip (which wasn't cheap). I felt good having it for the evacuation coverage. Apendicitis can strike anyone at any age. That's what I was thinking. I do not purchase it for the WDW portion of our vacations but definitely for the cruise portion as we are OUT of the country.
My Insurance DOES cover our family out of the country BUT our portion of a 25,000 evacuation cost would run $5000. That's after our insurance has paid 80%. Sometimes they pay 90% and that's still $2500!
I just found out our insurance also covers overseas and would cover an evacuation from the ship. I am still getting insurance though, simply incase one of us gets sick before the cruise and we cannot get on the ship. It is worth it to me to pay the $150 ( could be less now since i dont need medical) to cover the $4000 we paid for the airline tickets and cost of cruise and hotel.
We cruise every year with Disney and the 400.00 plus that we would spend on insurance would add up to a free cruise after a few years. Granted we travel in a cat. 10 room and cruise during the value season. I called my insurance and we are covered in all countries (my husband travels internationally for his job so we better be covered!) Just my 2 cents

If you buy it seperate from DCL it is MUCH cheaper and one of the companies covers kids for free, I forgot which one, but someone out here will help me. Like I said we also are covered out of the US but I buy it for the cost of the cruise and airline tickets. It is only going to be like $130 for us next time, well worth it to me for my peace of mind when one case of pink eye could cost me $4000.
If I didnt have to fly to get to the ship and it was only a 3 nighter MAYBE I wouldnt either. It would depend on how much it would cost to cover it. If you went with a company other than DCL it would probably be VERY cheap. On second thought, no I would still get it.
