Not sure where to post this (medical guardianship)....

We're (my DH and DD) leaving for WDW on Monday but my 15month old DS is staying home with Grandma : )
Does anyone know where I can find a form online that I can fill in and sign to leave with my mother in case she needs to take my DS for medical treatment?
She is listed on his pediatrician file to bring him in but if it's after hours and she goes to urgent care I'm afraid they will give her a hard time - and worse yet, if she needs to make any sort of medical decision I want her to be able to.

I don't think you can give over true medical guardianship. However you can probably write something up similar to the forms you fill out for school/daycare that gives her permission to authorize emergency medical care.

I would simply write a letter stating that you are going to be out of town and authorize _____ to obtain medical care for your child ____ in the event that you can't be reached. If you want to be extra safe have it notarized.

this is the permission to treat form that we used when my boys stayed with their grandparents while dh and I traveled to China to adopt my dd.

Whenever my husband and i take a trip away from the kids, or the kids take a trip away from us. I write a note giving the caretaker permission to authorize any and/all medical transportation and treatment, list the one child's drug allergy, list their doctors name, address and phone (also dentist) our cell phone numbers, and where we are staying and make a copy of our medical card. All this information is on the one sheet of paper

I have a document that I leave for my parents when they stay with the girls while we travel. I update it with their current weight, dosages for OTC meds, any prescriptions they are on, contact info for doctor/dentist/pharmacy/vet and a medical authorization form. It just states: I give permission for (name) or (name) to authorize medical treatment for (daughter/DOB) and (daughter/DOB). DD's drug allergies are also listed. Then I sign/date it. I also leave an insurance card.

I watched a friend's son for one day when her regular sitter was unavailable, she left me an authorization good for that one day, just in case. It's easy to print one off whenever.

I have heard that some states require a letter to be notarized by parent/guardian authorizing treatment. I have a standard letter w/my sons name, dob, allergies and medical conditions. It states that my parents (and i put their names) may make medical decisions between certain dates, for my DS.
I had actually heard that FL requires the letter to be notarized..my parents have taken my nephew and son to WDW and we did this..just in case.
