Dear rewardzcard holder,
We regret to inform you the rewardzcard prepaid program with Debix Systems, Inc will be discontinued. Any balances remaining on your account must be used by June 29 after which time a refund check will be mailed to you. Please accept our apology for any inconvenience this may cause you.
To avoid any future inconveniences, please read the important information below.
The card will be invalid after 5:00pm EST on June 29, 2001 and must be cut in half and destroyed. Please do not attempt to use your card after this date.
If there is a balance remaining on your card after June 29, 2001, within thirty (30) business days we will mail a refund check payable to you.
The refund check will be mailed to your most recent postal address as listed in our records.
Please don't forget to cancel any recurring charges for services paid for with the card (such as Internet Service Providers (ISPs), magazine subscriptions and the like). Failure to do so may result in your service providers levying late fees or other charges against you.
Cancellation of your card privileges will not affect your rights and obligations under the cardholder agreement, including amounts owed in connection with the card for fees, charges and overdrafts.
