I have been trying to get hold of one too! If anyone knows how to get one I'd be very grateful

I emailed Disney last week about the sponsorship form being out of date on their French website. I received a reply earlier today apologising and saying it had been passed to their internet team and should be updated in the next few days. If it is not updated by the end of the week I will be chasing it up as I could do with a new form as well.

Link to form in website in case anyone else is interested - although not much use at the moment as still showing form that expired on 7th November.
