Need Template for luggage tag

I want to create a Luggage tag for BLT. We are taking my mom there in June for her "last trip to the world". Now mind you, it's the 3rd time we are taking her for her "last trip to the world". She is 84 and other then needing a wheel chair, I think we will be taking her on a few more "last trips". But this one is special as we are staying in a DVC 1 bedroom castle view. Which means if she doesn't want to go out, she can sit on the balcony and look at her favorite place. I know there are many fine DISigners on this board who could make me a BLT luggage tag, but I wanted to do it myself for her. However, I can't find a blank template. So if someone would send me one I would greatly appreciate it. Thanks, Penny
