need special diet form WHERE?

Just booked our cruise!!! DCL told us we need to submit a speical diet form for DH. How do I get one of the special diet forms? DH due to medical reasons needs to eat little spices. (no Colon) Can someone help me?

I believe what they are referring to is the medical form in the "My Disney Cruise" section of www.disneycruise.com. That's where you fill out all of your forms, reserve excursions, etc. In the forms at the very bottom there is a medical form.
