Need help with letter for DS for school

DS's main Medical Specialist agrees 100% that he doesn't need to go to WDW with heavy crowds due to his anxiety disorder, ADHD, Sensory Issues, meltdowns, panic attacks, etc. The school Special Ed coordinator has already said she will OK our trip in Sept so that it doesn't count against DS as unexcused absences (we are in a very strict school district). Just to CYA, the Doctor said if I wrote a letter she will sign it so that the kids also have a MEDICAL EXCUSE for missing school.

Does anyone have a sample letter that they've used before? I've seen that long one that says how Epcot is a learning experience. That's not what I need. I'm looking for a letter written from a medical standpoint.
