Need Help w/ GM Discount Tickets

My dad (hourly retiree), my Stepdad (salary retiree) and my Stepsister (current employee) can NOT find the forms. They all tried the socrarates website and called HR. The HR people did not know what they were talking about. Does anybody have a copy of the forms or know exactly who to call to get the forms? Thank you for any help.

I don't have the number but you can get the forms from the GM lounge in Test Track- someone at Disney must know the number
