Need help from those who know.

I leave this Sat. for the World with 3 friends from work and I wanted to fax my room request and I can't find my example that I had with what I need to put on it. I've looked in Mousesavers and I can't find it!!!!! HELP. I know this should probably have gone on the Resorts board, (before some tells me that's where it should have gone) but this is the one I read the most so this is where I put it.
