Need Help - Fish Extender Gift Tags

Disney Dad hear. I have been tasked with creating fish extender gift tags to put on our fish extender gifts. I have spend the last two hours searching for templates or anything on the Dis Boards. Can anyone help me with this. We are going on the DCL Fantasy from 9/27 to 10/4.
