Need help filling out Bahamas form, anyone??

UPDATED:

I just need to know how to fill out the Bahamas visitor form.
since the ship is registered in the Bahamas we have to fill it out,

do I put Dreams Unlimited if i booked through them?
arriving by??? "other ship"
what is my date of arrival?? the day we leave port?
is my nights of stay -- 4

what about hotel? I am not at a hotel, do I leave this blank?

city where I boarded? is this Cape Canaveral?

No, there's a set of forms for each person. As I recall it includes:

Charging information (you fill out one per stateroom unless you want to have two accounts set up). On this form, you say who's authorized to charge.

Transportation information (one per person)

Bahamas immigration form (one per person). They were out of those for a while, and you had to get them at the port. And yes, you do this even if you don't go to Nassau, since 1) The ships are registered in the Bahamas, and 2) CC is in the Bahamas.

General contract info (one per person, and you can sign for minors).

I think there was an extra one if you were using the DCL buses, and you'd do one per person.

I don't remember what section 2 is, but I don't recall ever entering a travel agent's name on this form. I think for "arriving by" you enter the name of the ship. Date of arrival should be the day you call at the Bahamas, either Nassau day or Castaway Cay day - you can probably leave this blank and ask once you get to the port terminal. Nights of stay is 0, leave the hotel blank. Boarding city is Port Canaveral, I believe.

When and where do you get this form to fill out? Our cruise isn't until Sept and our TA is DU. Do they send it automatically as time nears or what? When can I expect it?

I just remember writing "The Magical Cruise Co, Celebration, FL" and for number of days, I just put "one".

We just received our docs for the June 9th cruise and the Bahamas form was in there. I believe I saw on another thread that you use Pt Canaveral as boarding city and Wonder as the hotel. Maybe someone who has been on the cruise before can tell us exactly. I'm going to get ours filled out this weekend when I have some time to read everything through.

We have cruised Disney twice before and I can't remember what we put then but I filled out the documents for our June 2 cruise last night and I did include DU as the Travel Agent.

If the forms are incomplete they will help you complete them at the port. I wouldn't stress over this.
