Need copy of settlement statement for taxes

Can someone tell me how to locate a copy of my settlement statement for my SSR purchases in 2004?

I bought two contracts (150, 100) through Disney and can only locate my BCV Disney settlement statement in my papers. Can anyone suggest the correct person and phone number. Thank you!

If you call the MS number it gives you an option to press for Member Accounting. I'd start there.
