Ok, I'm not hmgolden but i can answer your questions.
The photo session lasted about an hour, give or take. Maybe 1 1/2 hrs from hotel to hotel.
You get pictures in front of the castle (from whatever angles), inside the castle, inside the waiting room for CRT, on the balcony outside the castle, in the area behind the castle, and on the carousel. My understanding is that to have them almost anywhere else you had to pay. we wanted pictures on Dumbo, and they told us that would be an extra $2,000 or so (we didn't do it).
It was me, my husband and the photographer. Also, there were tons of maintenance workers tidying up the MK overnight, but they weren't with us
The photographer drove us up to the castle, and then drove us behid the castle for the pictures back there.
My dress didn't get dirty at all as far as i could tell. It was just spread out on the cement for the pictures.
I did not re-steam my dress, and it was fine. It would probably depend on the material.
