Medical Record?

We are moving next month out of state. My DS is 7 should I request his medical records from his Dr. to bring with me to his new Dr. I didn't think of this my mother did.

I sit here and wonder what else I am forgetting t do before we move. I feel like my brain is mush with everything I am trying to remember!
 
That might be a good idea if he has special problems, or you can wait until you have a new doctor.
 
Yes, request the medical records. We've moved several times and I've requested the records each time. And since we kept moving, it was much easier for me to have copies than the doctor's office to fax them to the new doctor's office. You will have to sign for them and they may charge $10 or so for them. I was told a few times there was a charge for them, but then they just handed them over. :confused3
 
Don't forget school records, or an assessment letter for the guidance counselor for proper placement. You don't want to get DS in the wrong grade. If you are moving to Florida, make sure you get the shot records. No child can be admitted to school without them. They have to go to the health department for transfer onto the "blue card".:confused3
 

Not just medical records, but immunization records too. You will need those to enroll your son in his new school. they should be a part of his medical records, but make sure you request them specifically. Also, review his medical records for any of the major things you may remember to make sure those transfer. Like an ER trip for a broken bone, etc.

If your son sees a specialist for any reason, a recommendation from his current specialist for one in the new area. For example, DD1 has epilepsy. When we moved, I asked her neurologist to recommend a neurologist in our new city. She did great, and we love the neuro here.

School records are important too. Especially if your son's birthday is near the cut off date.

Being military, we have moved several times. Moving took on a whole new dimension when we added the kids. So much more to think about!
 
Requesting your son's medical records can get costly. Out here its $15 access fee and then twenty five cents per page. If the record is being sent to another physican or facility, its done as a courtesy and no change is involved. Unless your son has a chronic problem that he will need to be seen by a physican right away, I wouldn't request them prior to moving. I would move, find a new pediatrician, then request that the records be sent to the new doctor. Generally it can take up to 10 working days to process requests and get them sent out. Its not normally something that can be done on demand at that moment, so that might be something else you will need to consider.
 
I just moved from OH to CA and was unable to have a driver's license issued to me under my married name, because I did not have my marriage certificate. If you are married or have changed your name for any reason, I would suggest bringing documentation. They destroyed my old ID from OH and I was issued the license under my maiden name and was told I could have it changed once I have the proper documents. I know it seems like a small inconvenience, but it has caused me more than a few headaches. I am unable to get a checking account as the name on my SS card and my license do not match. Any time I use my credit cards and someone asks for my ID, I have to explain the situation and usually results in having to pay cash. Even signing up for a Costco membership and my Disneyland AP has been a pain. Now I tell everyone who is moving out of state to have that marriage certificate with them!

I did get the immunization paperwork, and notified both schools.

Moving is so stresfull I can't stand it, especially moving out of state, I keep thinking of things I have to get done.
