Medical Form?

In my research I read that we need to bring a Medical Form with us when we check-in at the port. Where can I get this medical form?? I couldn't find it in my online reservation. Thanks!

I think you must be referring to the health questionnaire which they will send you before your cruise. It just asks if you or anyone in your party has traveled to any countries with Ebola or if anyone in your stateroom is having signs of sickness.

The medical form is given to you at check in. If it is busy someone may hand it to you when you are getting in line so you can fill it out while you wait.

Yep, what the PP said. They were handing them out in line when we checked in. Takes 2 seconds to fill out.

We got ours online. I think during the check in process. I have them all ready to fill out that day

thank you all - i was hoping to find it online - but i can't see it in the check-in process. I will try again. Maybe if I email them - they can send it to me. With 2 wiggly kids - managing to fill out all those forms in line - would not be a good start!

It's a very quick form. Just a couple of checkboxes indicating you are all well or whether anyone has had vomiting and fever in the past few days. Some cruises they provide it ahead of time, some have slightly different/additional questions, some will just give it to you at port.

They did send the health questionnaire via email for a short time. But found that people were answering the questions based on the day they were filling them out at home. And that 48 hour period being asked about is time-sensitive when boarding the ship.

They've returned to giving you the health questionnaire at the terminal again.
