Medical Form for Refrigerator

Where can I find this form? Or how do I get it? My son is taking medicine that needs to stay cold. My next question is will the ice box in the room keep it cold enough? And how do other people transport the medicine to the ship? My son has allerigies and he depends on his medicine. If any one can answer some of these questions for me it would greatly be appreciated.

It usually takes a while for the stateroom ice box to cool things. Guest Services has a refrigerator and they will store your medication for you if necessary, but I would rather keep my medication under my own control.


In the past, our travel agent has requested the form from DCL. I have my doctor fill it out and then send it in.

To keep your medicine cool in transit, pick up some small freezer packs that you can place in a small lunch bag and keep it in your carryon bag.

We used the Medical Information form that you can now download from the DCL website http://disneycruise.disney.go.com/dcl/en_US/myCruise/landing?name=PreArrivalPaperworkLandingPage

In the Current Medical Status and Medications area, we just put Needs refrigerator for daily medication and the refrigerator was in the room under the vanity when we arrived.

I got my form from my TA, but you can download the medical form on the DCL website. Have your Dr. fill it out and fax it along with a note requesting a fridge. They will provide a dorm room sized refrigerator. There are a limited number of refridgerators, so on the off chance that you can't get one, put ice in ziploc baggies and keep them in the cooler box in the stateroom
