I'm confused. Is 18% always added into the cost of food at Disney TS restaurants? If so, do we tip an additional 15-20%? I thought that I had read tips WEREN'T included in the price of meals..

No, the gratuity is up to you unless you are using the Tables in Wonderland dining card. Often with the dinner shows the gratuity is included in the price.
