You can call Disney's Magical Express Guest Services directly at (866) 599-0951.

They have always been extremely nice and very helpful to me.

I used this reservation form to add Magical Express to my package. It automatically generates a confirmation email, and then it appeared with my reservation information on MDE about a week later.

You aren't blind--I couldn't find it either so I finally called.

This form never worked for me, this year anyways, so I had to call and add my flight information. It has worked years previous though.

I can't get the form to work either - can't get past step 2 of entering my flight dates (I'm flying on 4/29/14). I just chatted online and the rep gave me the same link you have.

I got a little booklet from them about a week ago with luggage tags - which I thought would mean they'd go to Magical Express, though nothing in the booklet says anything.

I don't think Magical Express is automatically included when you make a reservation since Disney has no way of knowing if you plan to use that service vs. renting a car or having family pick you up from MCO. Last October, there was a page with our ME reservation information in our booklet, but I want to say they scanned our magicbands. We got two sets of luggage tags - One set was just decorative (had a place for contact info), and the other set was similar to what the airlines put around your luggage handles with a name, resort and barcode. That second set is the one needed to flag your bags for ME.

Since your trip is soon, I would recommend calling to confirm.
