Looking For Daily Planning Worksheet, not spreadsheet

Several trips ago, I had printed off daily worksheets from a link I found I think on the Disboards. They were a fill-in-the-blank type sheet, where you filled the day's agenda - the park you were going to, where you were dining and the ADR #, I believe there was a place for phone numbers. I think there was a page for travel information, like airline flight and confirmation numbers, rental car or transportation confirmation numbers, hotel confirmation numbers. It had pictures in the corners. It was not an excel spreadsheet that you filled in online.

Does anyone else remember this? Can someone send me the link or attachment? I know I found it from these boards, so someone must know what I'm talking about. TIA for your help!

That's not the one I was thinking of, but I Like It!!! I've saved it and will use it if I can't find the one I was looking for.

Anyone else know what planning worksheets I'm thinking of?

Have you tried Passporter.com? It is an e-worksheet and it is free to download. It is called Vacation At-A-Glance. Sorry I am not able to put the website on here so you can just clike it. How does everyone do all that?
